The information collected by the Birth Certificate department at Memorial Hermann is used solely to register the newborn baby with the Texas Department of State Health Services Vital Statistics.

Homeless Youth Birth Certificate
A Homeless Youth Birth Certificate is an official legal document. It expires six months from the date issued. A homeless youth 24 years of age or younger and born in Minnesota may request this certificate at no cost from any Minnesota vital records office.
The youth must complete the application found in the Youth Experiencing Homelessness Birth Certificate and provide identification. The youth may provide acceptable identification or submit a Statement to Identify or an original or a copy of a signed Affidavit of Homeless Status form. The Affidavit of Homeless Status is valid for six months after it is signed.

As a parent or legal guardian, you have to apply for your child. Both the parents have to appear in person and sign up at a passport agency or authorized passport acceptance facility. If only one parent can appear then a notarized Form DS-3053: Statement of Consent from the non-applying parent/guardian should be submitted. Both the parents are required to show their current valid ID. If you are not a U.S. citizen, you must show an alien registration card , valid foreign passport, or driverâs license. If you do not have an acceptable ID, someone with current valid ID must vouch for you as described above. A child would be issued a Passport valid for a maximum of five years.

How Do I Get A Birth Certificate If I Filled Out Paperwork At The Hospital/birth Center
If you have not submitted a request for a birth certificate by placing an order online, phone, mail, or in-person and submitted identification and proof of eligibility documentation, you will not receive a birth certificate.
New parents complete a Washington State Birth Filing Form at the hospital or birth center after delivery, not an application to order a birth certificate. The first birth certificate issued for a birth is NOT free and hospitals and birth centers cannot order a birth certificate on your behalf. Certificates Of Foreign Birth
Effective January 1, 2013, the Center for Health Statistics may prepare and register a Certificate of Foreign Birth upon receipt of a request from an adoptive parent or an adopted individual 19 years of age or older. A Certificate of Foreign Birth may be put on file for a child who has automatically acquired United States citizenship following a foreign adoption and who possesses a Certificate of Citizenship in accordance with the Child Citizenship Act. Upon receipt of the required documentation and the appropriate fee, the Center for Health Statistics will prepare a Certificate of Foreign Birth showing the childâs new name and the name of the new parent. Who Can Register A Birth

Fill Out The Certificate Of Live Birth Form
According to Sec. 1 of Presidential Decree 651, registering the birth of a baby is the responsibility of the attending physician, nurse, or hospital administrator.
The registration process begins soon after the mother gives birth to the baby.
You will be provided with a Certificate of Live Birth Form which is partially filled out with the date and hour of birth, name of the nurse and doctor, weight and sex of the baby, etc.
