    Many women find that their babies switch between breast and bottle and between breast milk and formula with ease. For others, there are hiccups along the way.

    In the late 19th and early 20th centuries, physicians came to understand that diseases were caused by germs and could be transmitted by consuming contaminated foodstuffs. In particular, raw milk, which spoiled readily , was found to transmit a variety of diseases, including tuberculosis, typhoid fever, cholera, and diphtheria.

    In 1864, Louis Pasteur discovered that keeping wine at a high temperature killed the bacteria that caused the wine to sour. The pasteurization process was employed by the dairy industry in 1890not to make milk “healthier” but to prevent milk transported in unrefrigerated railroad cars from souring. Several years later, it was discovered that pasteurization also protected against milk-borne diseases.3

    Many physicians vigorously opposed pasteurization, however, because they believed that the process significantly diminished the nutritional value of milk. In fact, pasteurized milk was found to be deficient in what were later identified as vitamins C and D, and children consuming pasteurized milk received daily doses of orange juice and cod liver oil to prevent scurvy and rickets. Pasteurization of milk became a universal practice in the US by about 1915.

    Composition Of Human Breast Milk

    Human breast milk is a complex matrix with a general composition of 87% water, 3.8% fat, 1.0% protein, and 7% lactose. The fat and lactose, respectively, provide 50% and 40% of the total energy of the milk . However, the composition of human breast milk is dynamic and changes over time, adapting itself to the changing needs of the growing child. For instance, during each nursing session, the milk that is expressed first is thinner with a higher content of lactose, which satisfies a babys thirst, and following the foremilk, hindmilk, is creamier with a much higher content of fat for the babys needs. Variations are also present with the stage of nursing , maternal diet, maternal health, and environmental exposure. During early lactation, the protein content in human milk ranges from 1.41.6 g/100 mL, to 0.81.0 g/100 mL after three to four months of lactation, to 0.70.8 g/100 mL after six months . The fat content varies significantly with maternal diet and is also positively related to weight gain during pregnancy. Remarkably, it has been observed that a mothers breast milk is almost always adequate in essential nutrients for her term infants growth and development, even when her own nutrition is inadequate. Although the mean concentrations of protein, sodium, chloride and potassium in early preterm milk are adequate to meet the estimated requirements for preterm infants, specific nutritional supplementation is required for mothers milk delivered to preterm infants .
