A Certificate of Foreign Birth may be placed on file in Oklahoma for minors born outside the US and who are adopted in Oklahoma by US citizens.

    Now that we have established how important a birth certificate for a baby is, were going to address how to get one for your newborn. Regardless of whether your child is scheduled to be born on a specific date or you are surprised with an unexpected delivery, you should carry your personal identification in order to register your childs birth.

    Usually, if you give birth at a hospital, they will provide you with the necessary forms before you are discharge to go home. Most often, a nurse or midwife will offer her assistance in completing the babys birth certificate form. If you have programed the birth at home or in a birthing center, a midwife generally has the required forms on hand. If you give birth outside a hospital is important to proceed correctly to create a certificate of live birth, and a correct register for a birth certificate.

    Births Through Assisted Reproduction Surrogacy Or With Three Or Four Parents

    Through the Childrens Law Reform Act, you may be recognized as a parent to a child in certain circumstances. For example, you may be recognized as a parent to a child:

    For adoptions which occurred after Nov 1, 1997, an uncertified copy of the original birth record may be obtained by the adopted person if:

  • The adopted person is at least 18 years of age AND
  • The identified birth parent did not sign Affidavits of Non-Disclosure AND
  • The subject of the adoption provides a signed notarized affidavit stating that he/she is unaware of the location of a biological sibling who under the age of 18 and currently living in an adoptive family .
  • The fee for the search and issuance of one certified copy is $15.

    For all other situations, the records are sealed and are not open for public inspection. In those cases, a certified court order from a court of competent jurisdiction is required to be submitted. The fee for the search and issuance of one certified copy is $40.

    What Types Of Birth Certifications Are Issued

    A computer-generated long form is the only version of birth certificate issued. Under South Carolina regulation, it is considered for all purposes the same as the original certificate.

    The computer-generated long form contains the following information:

    • State file number
    • Father’s name
    • Father’s place of birth
    • The date issued.

    Vital Records Offices stopped accepting requests for short form birth certificates in January 2015.A computer-generated long form can be issued by the state office location – DHEC, 2600 Bull Street, Columbia, South Carolina 29201 and by Regional Vital Records Offices .

    If You Have A Home Birth

    Complete and return the home birth worksheet within one year of the baby’s birth. If you do not return this form within a year of the child’s birth, the parent will need to provide additional evidence about the birth and pay an additional filing fee. More importantly, the birth certificate will be marked delayed. Government authorities question the validity of delayed birth certificates, and this could impact the child for his or her whole life.
