To request a Washington birth certificate, you are required to pay the state fees. From January 1, 2021, the cost to process a copy of a birth certificate in Washington is $25.
The same fee applies to certified and noncertified copies and each additional copy will require the payment of a $25 fee. Moreover, you should take into account the costs associated with shipping your petition.

How Do I Get A Birth Certificate If I Filled Out Paperwork At The Hospital/birth Center
If you have not submitted a request for a birth certificate by placing an order online, phone, mail, or in-person and submitted identification and proof of eligibility documentation, you will not receive a birth certificate.
New parents complete a Washington State Birth Filing Form at the hospital or birth center after delivery, not an application to order a birth certificate. The first birth certificate issued for a birth is NOT free and hospitals and birth centers cannot order a birth certificate on your behalf. The hospital may have provided you with your local county health department birth certificate order form, which requires a fee.

Who can request a birth certificate?
Births less than 100 years old are confidential and may only be issued to:
- Registrant if of legal age
- Parent listed on the Birth Record
- Legal representative of one of the above persons
- Upon receipt of registrants death record, a birth certificate marked Deceased may be issued to the spouse, child, grandchild, or sibling, if of legal age. Please notate Registrant Deceased on the application and include a photocopy of the death certificate.
VALID PHOTO IDENTIFICATION IS REQUIRED WHEN REQUESTING A BIRTH RECORD.
If applicant is not one of the above, the application must be accompanied with a notarized Affidavit to Release a Birth Certificate signed by one of the above, along with any supporting documentation and a copy of valid photo ID of both the person authorizing release and the applicant.
Ordering a Florida Birth Certificate
Any person who willfully and knowingly provides any false information on a certificate, record or report required by Chapter 382, Florida Statutes, or on an application or affidavit, or who obtains confidential information from any Vital Record under false or fraudulent purposes, commits a felony of the third degree, punishable as provided in Chapter 775, Florida Statutes.

How To Get Birth Records In Washington
Birth records in Washington can be obtained by applying in person or by mail to the Department of Health . Birth records are also available at the various Local County Health Departments in the state. In-person requests are processed and fulfilled the same day. In-person requests cost less than mail applications.
Requesters can obtain any of these three when applying for birth records in Washington:
- Birth Certificates: These are official documents of births used for legal purposes. According to the new amendments, only persons named in the certificates, their immediate relatives, or legal representatives are eligible to obtain birth certificates.

Effective January 1, 2015, pursuant to Senate Bill 1050, the Alameda County Clerk-Recorder’s Office will require the Notary Consumer Disclosure. This bill requires a legible notice at the top of the certificate of acknowledgment in an enclosed box, stating that the acknowledgment, proof of execution, or jurat verifies only the identity of the individual who signed the document to which the acknowledgment, proof of execution, or jurat is attached, and not the truthfulness, accuracy, or validity of the document, as specified below.
A notary public or other officer completing this certificate verifies only the identity of the individual who signed the document, to which this certificate is attached, and not the truthfulness, accuracy, or validity of that document.
Documents where the person appeared before a notary or other officer on or after January 1, 2015 must have this disclosure or the document will be rejected. Documents acknowledged before January 1, 2015 will be accepted for recording and must meet current applicable statue.
This requirement has been made in accordance with the legislative amendment to Sections 1189 and 1195 of the Civil Code, and to amend Section 8202 of the Government Code, relating to notaries public.

You must complete the following steps to request and receive a Heirloom birth certificate.
- Submit an application by mail or in person with required pieces of information.
- Provide documentation to prove your qualifying relationship.
- Pay applicable fee. Fees start at $50 per Heirloom birth certificate. Other fees may apply depending on shipping method. For each $50 purchase of an Heirloom birth certificate, $25 is tax-deductible.
